Medical Lake in Spokane County is a fly-fishing-focused trout lake managed under selective gear rules. Open from late April through October, it provides a quiet, no-gas-motor fishery for rainbow and brown trout using artificial lures and flies only.

WDFW stocks the lake each spring, and holdover fish can grow to 20 inches or more by fall. Fly anglers in float tubes or kayaks do well with leeches, chironomids, and dry flies during hatch windows. Spinning gear is allowed, but only with barbless artificial lures—no bait is permitted.

There is limited bank access, but a gravel launch on the southern shore allows for easy float tube and kayak launching. No motors are permitted, keeping the lake peaceful and ideal for quiet, technical fishing.

Though warmwater species exist, trout are the primary focus due to the lake’s regulations. Algae can reduce clarity in summer, but fishing remains productive into October.

Medical Lake is ideal for those seeking a low-pressure, regulation-driven trout fishery. Its restrictions help maintain quality fish and a peaceful environment, making it a favorite among fly anglers across Eastern Washington.
